Process for Client Assistance (using your fund)
- When you have enough money raised in your fund to give a scholarship to a client, you can fill out this client assistance request form, which will let us know you have a client in mind and how much that client will need from your fund.
- When we see that submission come in, we will send you an email to let you know that it is time for you to send our Financial Assistance Application to the client for them to fill out.
- Once we receive the client’s application, we will fill you in a bit more about the logistics of everything (signing case agreement, liability waiver, etc).
Process for Client Assistance (TYL general fund)
- If you haven’t raised any money in your account and still want to put someone on the waitlist for our general TYL fund, you can use that same form above. Please just keep in mind that we have 80+ providers and are trying to be as fair as possible, so we likely can only fund 1 person at a time.
- When we confirm that we have sufficient funding for your client, we will reach out via email to let you know that it is time for you to send our Financial Assistance Application to the client for them to fill out.
- Once we receive the client’s application, we will fill you in a bit more about the logistics of everything (signing case agreement, liability waiver, etc).
Notes:
- If you have a client in mind but don’t have quite enough in your account to fund the person’s treatment, you can still fill out the above form because we may be able to pull from our general TYL fund to help out.
- We can’t promise this, but if the amount needed is rather low (<$1k), it is of course easier for us to find that money and make it work compared to if the person needs $2,000-$4,000 from us.
- We ask that you don’t share the application form with any clients until we confirm it’s OK to do so. The last thing we want to do is open up the floodgates of applications, knowing we don’t have enough money to help everyone.
